Anti Smooth Muscle Actin antibody does not stain cardiac or skeletal muscle; however, it will stain myofibroblasts and myoepithelial cells. This antibody could be used together with Muscle Specific Actin to distinguish leiomyosarcoma from rhabdomyosarcoma. In most cases of rhabdomyosarcoma, this antibody gives negative results whereas Muscle Specific Actin is positive in the rhabdomyoblasts. Leiomyosarcomas are positive with both Muscle Specific Actin and Smooth Muscle Actin antibodies.

This antibody reacts with a formalin-resistant intracytoplasmic epitope found in the majority of B cells and now considered to be the CD20 antigen, a pan-B cell marker. The antibody primarily recognizes a 33 kD polypeptide B cell component and also a minor 30 kD cellular antigen. The staining pattern is consistent with pan-B reactivity, producing staining for B cells in lymphoid and peripheral blood tissue. This antibody intensely stains germinal centers and B immunoblots in lymphoid tissue. The L26 antibody is widely regarded as the most selective B cell marker currently available. L26 has shown no cross reactivity with nonlymphoid tissues and there are several studies demonstrating its sensitivity and specificity in formalin fixed, paraffin-embedded tissues. Studies have demonstrated that, in cases of lymphocyte predominant Hodgkin’s lymphomas, L&H variants of Reed-Sternberg cells show a strong L26 reactive pattern that is distinctive from other Reed Sternberg variants. L26 may prove to be a useful marker for this particular subset of Hodgkin’s lymphomas.

S-100 belongs to the family of calcium binding proteins. S100A and S100B proteins are two members of the S100 family. S100A is composed of an alpha and beta chain whereas S100B is composed of two beta chains. S-100 protein has been found in normal melanocytes, Langerhans cells, histiocytes, chondrocytes, lipocytes, skeletal and cardiac muscle, Schwann cells, epithelial and myoepithelial cells of the breast, salivary and sweat glands, as well as in glial cells. Neoplasms derived from these cells also express S-100 protein, albeit non-uniformly. A large number of well differentiated tumors of the salivary gland, adipose and cartilaginous tissue, and Schwann cell derived tumors express S-100 protein. Almost all malignant melanomas and cases of histiocytosis x are positive for S-100 protein. Despite the fact that S-100 protein is an ubiquitous substance, its demonstration is of great value in the identification of several neoplasms, particularly melanomas.

Anti-Cytokeratin, 34beta E12 shows no reactivity with hepatocytes, pancreatic acinar cells, proximal renal tubules or endometrial glands; there has been no reactivity with cells derived from simple epithelia. Mesenchymal tumors, lymphomas, melanomas, neural tumors and neuroendocrine tumors are unreactive with this antibody.
